Initial Registration:

Those
who will register are required to bring the following:

1. duly
accomplished Oath Form or Panunumpa ng Propesyonal

2. current Community
Tax Certificate (cedula)

3. 2 pieces passport size picture (colored with
white background and complete nametag)

4. 1 piece 1” x 1” picture
(colored with white background and complete nametag)

5. 2 sets of metered
documentary stamps, and

6. 1 short brown envelope with name and
profession;

7. and to pay the Initial Registration Fee of P600 and Annual
Registration Fee of P450 for 2008-2011. (total: Php1,050)

Successful examinees should
personally register and sign in the Roster of Registered Professionals.

The oathtaking ceremony of the successful examinees in the said
examination in Manila as well as the previous ones who have not taken
their Oath of Professional will be held before the Board on Tuesday and
Wednesday, August 19 and 20, 2008, with morning (8:00 A.M.) and
afternoon (1:00 P.M.) sessions at the SMX Convention Center(Yep, dun ulit MAGIS!), SM Mall of
Asia, Pasay City.

Dress Code:

> All must come in their white gala uniform

> nurses
cap (females of course)

> white duty shoes (our Pinning Shoes, for girls yung may heels)

> without earrings

> hair not touching the collar (Guys, new haircut and Girls, wear your best bun)

> and without corsage (just your MALTESE CROSS)

Admission tickets for the Manila oathtaking
will be available starting August 1, 2008 in your respective colleges
of Nursing. Walk-in inductees who wish to join the Manila oathtaking
ceremonies may secure admission tickets at the Philippine Nurses’
Association, 1663 F.T. Benitez Street, Malate, Manila on a strictly
first come first serve basis.

Verification vs Certification

• Verification started on monday (July 28) at the Lepanto gate of the Polytechnic University of the Philippines (PUP). This is mainly for those whose names did not appear on the list of passers (internet, newspapers). Successful examiness could also verify the results if they want to. Your board rating and breakdown of grades will be written on a piece of paper (Note: This does not replace the Certification of Passing/Rating/Placer) (Requirements: Bring NOA and a valid ID)

• Certification of Passing/Rating/Placer will start on August 4 (Monday). With this, you will be able to get a ‘legal’ copy of your rating/certification that you passed and/or placed in the NLE.

Therefore, if you just want to know your rating, you can go for verification. But if i were you, i would wait for the certification on August 4, where you would be able to know your rating and receive a certification as well.

These are the requirements for certification:

Regular Fee:

1. A fee of Php 75.00 for each certificate
2. Metered Documentary Stamps (Available at the Customer Service Center)
3. Bring NOA and a valid ID

Express Lane (Same-day release):

1. No Representatives except if with proper ID and Authorization letter
2. Release of certification requests start at:

• 11AM  - for those who filed request from 8AM to 9:30AM
• 3PM - for those who filed request from 9:31AM to 2:00PM

     3. Requests received beyond 2:00 PM shall be released at 8:30AM 
        of the following day.
     4.  Certification Fee for express lane is Php 200.00/copy
     5. Bring Notice of Admission(NOA-yung blue form na ginamit sa
         board exam)  and a valid ID (school, driver’s license, passport)

"Pakisabi sa Magis sked ng oath taking nila August 19, PM.. 1 pm SMX , so dey hav 2 b der b4 1 pm, lahat na, it’s not alphabetical anymore.. lahat ng 338.."

Batch MAGIS will be taking the oath on  AUGUST 19, 2008 in the AFTERNOON (1PM) as a whole batch. You can buy the tickets directly from TUA-SLCN starting monday (august 4).

Board Passer = Php350.00

Guest(1 only) = Php 250.00
